For safety reasons, freshman physical exams and immunization records need to be submitted to the LFHS nurse's office by the first day of school. In addition, all new students to Lake Forest High School District 115 will be required to present proof that an eye examination was performed within the previous year by October 15 of their registration year.

Please make sure your freshman student's physical and immunizations are up to date and submitted to the nurse's office by Tuesday, August 1, 2017.All medical and health related forms can be mailed in, faxed to 847-810-1399, emailed to dbower@lfschools.netor turned in directly at the high school.

Doctor appointments fill up quickly, so schedule your child's exam as soon as possible. DO NOT TURN INCOMING FRESHMAN PHYSICALS INTO THE ATHLETIC OFFICE. The Health Office will forward a list of all compliant students to the Athletic Office in time for the first day of sports. Please direct any questions to Nurse Diane Bower, R.N. at dbower@lfschools.net.
