D67 Registration Deadline: Monday, June 24
District Families,
A personalized SnapCode email to complete online registration and fee payment for the 2019-20 school year has been sent to all currently enrolled District 67 families.
We encourage families to complete online registration as soon as possible. Completing your online registration and fee payment by the Monday, June 24 deadline will save families $25 in registration costs per student. Your student's registration will not be completed until you have submitted the online registration and fee payment. You will receive a confirmation email after submission.

Families new to the District will receive their SnapCode email upon completion of the verification of residency and enrollment process. Information about this process can be viewed on the Registration Page of the district website.
School Supplies
School Supplies
The approved School Supply lists can be found on the Registration Page of the District website. Please refer to these lists when purchasing your supplies, including supplies for classes other than grade-level (i.e. World Language, Band, Art, etc.).
The APT no longer sponsors a school supply purchasing program, but has provided options to assist families who wish to purchase supplies online (view the flyer here). Please note: These links are being provided as a convenience and for informational purposes only; they do not constitute an endorsement or an approval by District 67 or the District 67 APT.

Health Requirements and Forms
A physical exam must be completed within one year prior to preschool, kindergarten, sixth grade, and for new students entering the District from another state or country.
  • The exam must be recorded on the Certificate of Child Health Examination form;
  • the physical exam form signed and dated by your health care provider with the name, address and telephone number of your health care provider clearly written or stamped on the form;
  • the health history section of the form completed, dated, and signed by the parent;
  • immunizations completed according to the schedule of the Illinois Department of Public Health; and
  • all sections on the Certificate of Health Examination completed (front and back).
A dental exam is required for kindergarten, second, and sixth grades and an eye exam for kindergarten, or for any student enrolling in an Illinois school for the first time.
All forms are due in the school office by the first day of school.
